Clinical Study of Changes in Blood Preseeure of Hypertensive Patients in the Perioperative Periods


Abstract
Hypertensive patients with or without the associated pathophysiological changes have led to many unexpected problems during the perioperative periods. Because the exact causes of the essential hypertension is still uncertain and there are many factors influencing on the arterial blood pressure, the patients may not be cared well easily in the perioperative periods.
The authors studied retrospectively how the possible factors influenced on the arterial blood pressure in the hypertensive patients during the perioperative periods by the observation of hospital charts of 2084 elective cases from Aug. in 1975 to July in 1977.
The results are as follows;
1) The incidence of the essential hypertension in Korean people who have been operated electively was 6.3% (131 cases of 2084).
2) In the hypertensive group, the arterial blood pressure was decreased in the range of 10^15% from after-premedication to the postoperative periods.
But in the recovery period of anesthesia, the degree of decrease in the arterial blood pressure seemed to be recovered somewhat, and thereafter it was shown to be re-declined.
3) All inhalation anesthetics have properties to lower the arterial blood pressure regardless
of the hypertension, but there were no significance between the anesthetics.
4) On the basis of our results, we suggest that the hypertensive state can be managed without severe complications in the perioperative periods by the administration of proper premedicants and adequate preoperative visits.
